General Practice Referrals
Referral Instructions: Please click on the Provider Referral Link. And complete the form. It will ask for your name and email to enter into the document. Complete the referral form and a copy link will be sent to your email for the patient’s chart. Our office will contact the patient within 24 business hours and follow up with your office.
How do I make a referral?
Click on the URL for the Docusign referral form. You will provide your name and email and complete the form. Once completed, our office will receive an email indicating there is a referral on the Docusign website and a second copy link will go to you, the provider’s email, for the file copy. One of our clinical assistants will contact the patient and keep your office informed if we are unable to schedule an appointment.

What insurances do you take?
We accept most major insurances including Tricare, Anthem, Optima, United Health Care, Aetna, Medicare, Magellan, Humana, Beacon, Medicaid products, and Catasys.
Do you participate in EAPs?
We participate in some EAPs. Please call our office to confirm we accept your patient’s EAP.
What if we don’t accept your patient’s insurance or they don’t have insurance?
You have the opportunity to see our licensed clinicians at self-pay rates. We also have residents on staff to be seen at a reduced self-pay rate.
What clinical services do we provide?
Our clinicians provide individual, marital, and family therapy as well as psychological testing. We have clinicians that utilize Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Radically Open Dialectical Behavior Therapy (RO-DBT), Art Therapy, EMDR, and DBT Prolonged Exposure. We have one therapist that specializes in Bowenian Family Systems.
What issues do you specialize in?
We offer services for depression, anxiety disorders including OCD, panic, social anxiety, complex trauma, suicidal ideation and/or self-harm, borderline personality disorder, PTSD, school and conduct problems, adjustment difficulties, anger management, co-occurring substance abuse, military-related issues, Asperger’s (Autism Spectrum Disorder), post-partum issues, women’s issues, grief and loss, family and relationship problems, and psychological testing.
Do you offer psychological testing?
We currently have a Resident in Psychology on staff offering psychological testing at self-pay rates. Please note that the COVID restrictions affects availability of some testing.
Do you provide family or marital counseling?
Yes, we have a limited number of clinicians that specialize in family and marital counseling.
